NSC Monumentous Miniature Golf
Tournament and Fund Raiser

Saturday May 14, 2011 beginning at 8am

There are 18 regular miniature golf holes and one "BONUS HOLE-IN-ONE" 19th hole in this custom-build course by Andersen Construction Company winding its way throughout the Northshore Senior Center and Health & Wellness Center in Bothell.  A special Pancake Breakfast will be available from 8am until 11am for $4 (included in your registration fee if you sign up for the Early Bird package). Snacks will be available throughout the course from 11am until closing.  JAMBA JUICE!! Yes, a limited supply (750 portions) of free Jamba Juice will be given out between 11am and 4pm. 

We have golf packages for everyone.  Tee times are every 10 minutes beginning on the hour, and have four players for every tee time.  If you don't have a group of four, you can be assigned to play with other individual players who select a similar tee time. Register online to reserve your tee time, download the registration form, or contact Jason Cummings (NSCmarketing@seniorservices.org).

Great prizes for top three groups of four players

All participants will receive a free souvenir golf ball, a free raffle ticket to win a great gift, and an opportunity to win prizes on the course.

Golf Hole Descriptions:

Holes 1 through 8 are in the NSC Main Activity Building; Hole 9 goes across the Skybridge, and Holes 10 through 19 are at the Health & Wellness Center Building.

• Hole 1 – Our “Starter” hole is in our Multi-Purpose Room (MPR) near the Dining Room, where we will be serving breakfast and players will wait for their tee time. Sponsored by Williams and Williams.
• Hole 2 – Our “Stage” hole in our MPR will be front and center. Sponsored by Riverside East
• Hole 3 – This “Door” hole will have spectator viewing as it winds along the MPR’s open outside doors (if weather permits). Sponsored by Aegis Living 
• Hole 4 – This “Garden” hole takes you through the garden and will be in viewing of our coffee bar crowd. Sponsored by Chateau at Bothell Landing.
• Hole 5 – This “Crafts” hole starts outside in the garden, then turns into one of our craft rooms. Sponsored by Merrill Gardens at Mill Creek and Merrill Gardens at The Creekside
• Hole 6 – This “Amen Corner” hole is at the far turn of the first floor in the Senior Center, near our back entrance. Sponsored by US Bank.
• Hole 7 – This “Tech” hole plays through our computer learning center, computer workstations and all.
• Hole 8 – This “Pool” hole plays through our billiards room on the second floor of the Senior Center. Sponsored by Cox Printing & Signs 
• Hole 9 – This “Monumentous” hole is across the bridge, heading to the Health & Wellness Center. Sponsored by Andersen Construction 
• Hole 10 – This “Stairs” hole starts on the third floor of the Health & Wellness Center, down a stairwell, and finishes a floor below. Sponsored by North Creek Retirement 
• Hole 11 – This “Beauty” hole winds through our personal care parlor -- chairs and all.
• Hole 12 – This “Transportation” hole curves through our transportation dispatch center.
• Hole 13 – This “Therapy” hole brings the players through our therapy room, which they may need at this point. Sponsored by Visiting Angels Eastside
• Hole 14 – This “Workout” hole takes you around our equipment in the exercising room.

• Hole 15 – This “Cool down” hole completes the journey through the equipment in the exercising room. Sponsored by Life Care Center of Kirkland
• Hole 16 – This “Elevator” hole plays through the elevator, down to the main floor. Sponsored by ThyssenKrupp Elevator Americas
• Hole 17 – This “Almost Done” hole starts at the entrance of our Health & Wellness Center and through the small activities room, where spectators seating is located. Sponsored by Senior Helpers
• Hole 18 – This “Finishing” holes goes through our kitchen into the large activity, the location for drinks, food, and fun. Sponsored by Evergreen Washelli at Bothell.
• Our Extra Hole 19 – This special hole is located outside the large activity room for an opportunity to win a special prize in front of our spectators. 32" TV prize and hole sponsored by Foundation House at Bothell.

SPONSORSHIP BENEFITS ARE:
1. Name on event marketing materials such as flyers, posters.
2. Name and logo on signs at Tee-Off and Hole locations, and on signage at various locations at NSC
3. Name and logo on NSC website pages advertising this event
4. Two company people on course (at Tee-Off and at hole) may wear badges, clothing indicating company. See NOTE below about restrictions on soliciting participant or observer contact information.
5. Donor’s marketing materials (Pamphlets, brochures) may be displayed on designated literature table shared by all sponsors and in-kind donors at event

SPONSORSHIP RESPONSIBILITIES ARE:
1. Provide logo image in JPG format with minimum 200ppi resolution for marketing materials and website.
2. Provide 2 people as “Hole Marshalls” to help direct participants or observers along the course

NOTE: Sponsor personnel may NOT solicit business and may not gather contact information from participants or observers, but may answer any questions initiated by participants or observers. Sponsor personnel may carry business cards, but may only give them out if directly requested by participants or observers.
