Senior Wellness Project... |
| The Senior Wellness Project is an initiative
to replicate three innovative programs--Lifetime Fitness, Self-Management of Chronic
Conditions, and the Health Enhancement Program, which includes Health Mentors. The
goal of the Senior Wellness Project is to widen the access of older adults who face
limitations in their activities of daily living to this low-cost, high-quality and truly
comprehensive system of health promotion programs. |
| In the past four years, Senior Services, the
largest non profit agency serving seniors in Washington state, has had the opportunity to
test a series of research-based interventions designed to improve the health and
functioning of older adults with chronic conditions and to reduce unnecessary medical care
utilization. The programs were shown, in randomized, controlled trials to markedly
improve physical and emotional functioning, activity levels and self-efficacy.
Moreover, the Health Enhancement Program reduced hospital days by 72%. |
| The Senior Wellness Project proceeds in three
phases. In phase one, locations and client groups are prioritized, ensuring ethnic,
cultural and geographic diversity and access. Each site creates a work plan for
implementing phase two and three. The goal of phase two is to develop the Lifetime
Fitness and Self-Management of Chronic Conditions classes, from recruiting and training
instructors to providing classes on an ongoing basis. Once these programs are in
place, phase three begins which is the development of the Health Enhancement
Program. This includes hiring and training nurses and social workers along
with recruitment and training of Health Mentors. |
| The Senior Wellness Project staff works with
site staff in implementing the programs. A memorandum of agreement is signed and
staff from each site then develop a work plan including tasks to be accomplished,
timelines in which to accomplish them, and persons responsible for each task.
Quality assurance activities are conducted on all programs of the project. |
| We are extremely excited to be able to help
in bringing these successful programs to centers and sites throughout our region. For more
