Family Caregiver Program

The Northshore Family Caregiver Support Program (FCSP) has two goals:
~ To support caregivers in managing their care receivers’ chronic conditions, focusing on caregiver self-care and increasing caregiver competencies and skills to better handle situations, emotions and address future changes; and
~ To work with the care receiver and caregiver jointly and separately to improve communication and self-care behaviors, so that they can remain together for a longer period of time, with the goal that both will experience less stress, burden and physical strain.
As a site of the National Family Caregiver Support Program for both King and Snohomish Counties, we have a rich variety of supportive programs and services to help, including:

Caregiver Support Groups
- Caregiver Trainings - Powerful Tools For Caregivers*
- Respite opportunities
- Individual or family consultation and counseling
- Information and referral to other programs and services
*Powerful Tools For Caregivers is a 6-week educational series for family caregivers, in which the focus is on the self-care of the family caregiver. In the interactive sessions, you will learn "tools" to help you take care of yourself.

TCARE® Personal Caregiver Survey
A first step in identifying your needs as a caregiver is to complete a Tailored Caregiver Assessment and Referral (TCARE®) Personal Caregiver Survey. This survey will ask you to reflect on your experiences as a caregiver and rate your responses to a series of statements. This survey will help you and a Caregiver Specialist better understand your current caregiving situation.
You can complete the survey on your own using the form links below and mail or email your completed survey to Janet Zielasko, Northshore Senior Center Social Services Manager, 10201 E. Riverside Dr., Bothell, WA 98011 or
janetz@seniorservices.org. You can also contact Janet at 425-286-1035 to schedule an in-person or telephone appointment with a Caregiver Specialist to complete the survey together.
After completion of the survey, you and a Caregiver Specialist will review the results and work together to identify programs and resources that are tailored to your current needs.
Completion of this survey is not required to access caregiver support programs but research by the survey developers, Rhonda Montgomery, Ph.D. and colleagues at the University of Wisconsin, show that this survey accurately assesses caregiver needs and facilitates efficient delivery of caregiver support services.
